Job: Licensed Practical Nurse (LPN)

POSITION SUMMARY:

Under the direction of the Medical Director and within the scope of practical nursing, the LPN provides nursing care, medical treatment, and helps facilitate the provision of behavioral health services for clients at an outpatient behavioral health clinic for children. The LPN performs administrative tasks such as updating client records, obtaining medical histories, maintaining interface with the Lab, and providing scheduling guidance to medical department staff.

SPECIFIC RESPONSIBILITIES:

•Work independently under the supervision of the Medical Director.
•Receive and process all requests for medication refills utilizing nursing log: locate chart, request information from other sites if applicable, determine if medication information is correct and fill out medication refill progress note, after approval by Doctor, notification of pharmacy/ family that prescription is ready for pickup.
•Triage phone calls to the Doctors, i.e.: Medication side effects, crisis calls and any other patient concerns. Contact Doctor as needed and follow and document orders.
•Aid with the maintenance of medical records on all clients, including generating and following up on incident Reports regarding injury, medication adverse events or other incidents pertaining to medication.
•Obtain consent for new medication from legal guardian if physician is unable to obtain signature at time of appointment.
•In coordination with the Medical Secretary, organize forms in the Medical Department, update and copy forms as needed by Doctors.
•Assist physician with Prior Authorization Process.
•Obtain Vital Signs for appointments at Physician discretion.
•Perform chart audits to keep up to date on status of psychiatric evaluations, labs, AIMS testing, or other tests ordered and pass information to the Medical Office Assistant to make doctor aware of need.
•Obtain laboratory results for tests taken and route appropriately to the psychiatrist or pediatrician as determined by any abnormalities noted using knowledge of adolescent lab values.
•Accurately document in notes when a new medication is started, when patient education is complete, noting any side effects or adverse reaction and any changes in medications.
•Properly inventory and check in all samples, dispose of expired samples, and record such activity.
•Provide medical orientation to new staff and offer guidance and support.
•Meet with patients and families as per antidepressant protocol.
•Perform other duties as assigned that are consistent with the general duties of the position.

QUALIFICATIONS:

Education* & Experience:
Must possess and maintain a Licensed Practical Nurse (LPN) certification in the State of Arizona.
Prior experience working with children in the behavioral health field preferred.
Must obtain and maintain certifications in CPR, first aid, therapeutic crisis intervention (TCI).

Must be at least 21 years of age and able to obtain a Class I Fingerprint Card.
